
The new Connecticard Monthly Report form for Connecticut public libraries has moved.
The new report form link is below. You will need to have the new user name and password sent to the library's director in order to access the form..
Form Instructions:
- Click on the link provided below and type in user name and password and submit
- Click on “2011 Connecticard – Continue Survey” and choose the available report link
- Report form opens in new window (sometimes slowly, so be patient!)
- Click into appropriate boxes to enter numbers for the month and then click “Save” or hit “Enter”
- Use the print icon to print (optional) before closing the window
- Logoff
“Certification” is at year-end only and is no longer done monthly.
